On behalf of our team of caregivers and support staff, thank you for considering our healthcare center. We offer a full range of medical services to meet the individual needs of our residents, including short-term rehabilitative services and long-term skilled care. Working with the resident’s physician, our staff – including medical specialists, nurses, nutritionists, dietitians and social workers – establishes a comprehensive treatment plan intended to restore you or your loved one to the highest practicable potential. Our goal is to provide quality care in a comfortable, compassionate environment, demonstrating on a daily basis our guiding principle of providing Quality Through People.

Overview
Fifth Avenue Healthcare Center provides both short-term rehabilitative care and long-term skilled nursing. Our combination of clinical skill and comprehensive rehabilitation services enables us to care for a variety of medical conditions. Clinical services include: IV therapy, pain management, enteral nutrition, diabetes management and complex wound care. Rehabilitative therapies are provided as needed by physical, occupational and speech therapists. Fifth Avenue Healthcare Center also features an attractive, home-like environment that includes a beautifully appointed formal dining room and a lush courtyard frequently used for barbecues and parties. We can accommodate 54 residents. We have caring professionals who strive to meet the needs of our residents. Above all, we pride ourselves in helping our residents achieve their fullest practicable level of functional ability and independence. Family and friends are encouraged to visit frequently and are welcome to become a part of our extended family.
